This form signed needs to be filled out by the sub-consultants in order to give them access to our systems.  


Also, a service desk ticket needs to be created for tracking purposes for the sub-consultants to be set up with the accesses.  Please create tasks for the necessary access (such as email address, Active Directory account, Jabber, WebEx, O365, etc.).  


We need to also send a copy of the signed form to Ed Benes in the Legal department.
